Job Objective(s):

The Personalized Activities Specialist is responsible for assisting with the day to day delivery of resident centered activities and programs for the community while meeting and/or exceeding Gentle Touch Health Initiatives quality standards.

Under the supervision and direction of the Activities and Volunteer Coordinator, the Personalized Activities Specialist will be responsible for working with residents that have high energy creating centered focused activities (physical, spiritual, social and emotional).

Job Responsibilities and Duties


Essential Duties: Activities Program

  • Assists in maintaining a balanced resident centered activity program (physical, spiritual, social and emotional) for the Cottagewood community following the Activity Program Guidelines, Programming Guides(s) and direction from immediate supervisor
  • Conducts and/or assists with planned resident activities, programs and services from one on one up to 32 residents
  • Assists, invites and encourages residents to participate in activities
  • Assists residents to and from the activity areas when necessary
  • Records and documents resident participation per Gentle Touch Health Initiatives quality service standards, federal, state and local laws and regulations
  • Maintains activity areas in an orderly manner
  • Assists in maintaining an inventory of activity and programming supplies, games, programs and craft services
  • As applicable, responsible for daily care of any animals and/or plants within the activities program and services


Essential Duties: Volunteer & Community Focus

  • Works with volunteers and oversees their activities as needed
  • Involves families, residents, and community volunteers to participate as a regular part of the activity program as directed by immediate supervisor


Essential Duties: Quality Assurance and Regulatory Compliance

  • Demonstrates proficiency and understands essential quality service standards pertaining to enriching resident centered activities and programming; partners with community team in striving for excellent quality care & service
  • Partners with community team to ensure community follows all federal, state, local laws & regulations & Gentle Touch Health Initiatives quality standards for resident care and services
  • Maintains and/or exceeds standards of cleanliness, hygiene, proper attire and health standards


Essential Duties: Resident Focus

  • Reviews, reads, notates and initializes the Daily Log and Department Log to document and learn about pertinent information and any resident’s physical and behavioral changes as well as activities information
  • Practices positive resident relations; responds to resident requests and directs resident feedback and requests to immediate supervisor
  • Reviews the ISP (Individualized Service Plan), Resident Profile and Addendums for every new resident
  • Maintains and protects the confidentiality of resident information always
  • Essential Duties: Risk Management and General Safety
  • Partners with community team to ensure community is following OSHA requirements and promotion of Risk Management programs and policies; adherence to safety rules and regulations
  • Practices safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, MSDS (Material Safety Data Sheets) and Lockout/Tagout procedures

  • 2 year degree preferred or equivalent of 4 years’ experience in the geriatric
  • Ability to work semi-independently
  • Ability to lead activities with residents that have high energy and challenging behaviors
  • Ability to handle multiple priorities
  • Possess written and verbal skills for effective communication and the ability to facilitate small group presentations
  • Competent in organizational, time management skills
  • Demonstrates good judgment, problem solving and decision making skills
  • Ability to make responsible choices and decisions and act in a resident’s best interest
  • Ability to work semi-independently without direct supervision by following community procedures and guidelines and can follow through on assigned tasks
  • Demonstration of proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel), with the ability to learn new applications


Online Training

  • Fulfills all required training within the required timeframe, including the completion of all online training and study application appropriate for the position.
  • Completes all training required by the state or other regulating authorities including ongoing training per regulations
