    The  Personalized Activities Specialist/Activities Assistant is responsible for assisting with the day to day delivery of resident centered activities and programs for the community while meeting and/or exceeding Gentle Touch Health Initiatives quality standards.

    Under the supervision and direction of the Activities and Volunteer Coordinator, the Personalized Activities Specialist/Activities Assistant will be responsible for working with the residents on the campus that have high energy creating centered focused activities.


    • Assists in maintaining a balanced resident centered activity program for the Cottagewood community following the Activity Program Guidelines, Programming Guides(s) and direction from immediate supervisor
    • Assists in preparing the activity calendar and newsletter with the coordination of immediate supervisor
    • Conducts and/or assists with planned resident activities, programs and services throughout the community
    • Assists, invites and encourages residents to participate in activities
    • Assists residents to and from the activity areas when necessary
    • Records and documents resident participation according to Gentle Touch Health Initiatives quality service standards, federal, state and local laws and regulations
    • Maintains activity areas in an orderly manner
    • Assists in maintaining an inventory of activity and programming supplies, games, programs and craft services
    • Assists with the network contacts and relationships, and provides assistance in arranging for speakers from community groups/organizations to provide educational, social and engaging activities and support for the residents
    • As applicable, responsible for daily care of any animals and/or plants within the activities program and services

    Essential Duties: Volunteer & Community Focus

    • Under the leadership of immediate supervisor, assists with and supports the networking actions for retaining a volunteer base, maintaining a relationship with local businesses, organizations and schools
    • Assists with the volunteer orientations, volunteer training programs and appropriate volunteer paperwork
    • Works with volunteers and oversees their activities as needed
    • Involves families, residents, and community volunteers to participate as a regular part of the activity program as directed by immediate supervisor



    Essential Duties: Quality Assurance and Regulatory Compliance

    • Demonstrates proficiency and understands essential quality service standards pertaining to enriching resident centered activities and programming; partners with community team in striving for excellent quality care & service
    • Partners with community team to ensure community follows all federal, state, local laws & regulations & Gentle Touch Health Initiatives quality standards for resident care and services
    • Maintains and/or exceeds standards of cleanliness, hygiene, proper attire and health standards
    • As applicable, all employees who drive a DOT Transportation Vehicle, must review and sign the Driver job description and understand the key essential duties for safety and regulatory compliance and successfully complete the Driver Training

    Essential Duties: Resident Focus

    • Reviews, reads, notates and initializes the Daily Log and Department Log to document and learn about pertinent information and any resident’s physical and behavioral changes as well as activities information
    • Practices positive resident relations; responds to resident requests and directs resident feedback and requests to immediate supervisor
    • Reviews the ISP (Individualized Service Plan), Resident Profile and Addendums for every new resident
    • Maintains and protects the confidentiality of resident information at all times
    • Essential Duties: Risk Management and General Safety
    • Partners with community team to ensure community is in compliance with OSHA requirements and promotion of Risk Management programs and policies; adherence to safety rules and regulations
    • Practices safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, MSDS (Material Safety Data Sheets) and Lockout/Tagout procedures

    • Committed to serving our residents and guests
    • Contributes in the overall engagement programs and processes (customer and employee engagement) including participating in the employee engagement survey and engagement improvement planning workshops
    • Attends regular meetings; Town Hall, Department Team Meetings, and others as directed by the Supervisor / Department Coordinator 


    Online Training

    • Fulfills all required training within the required timeframe, including the completion of all online training and study application appropriate for the position.
    • Completes all training required by the state or other regulating authorities including ongoing training per regulations
    • Completion of the following training may be required and/or certificates available per regulations and laws as applicable:
    • Driver Training


    • To perform this job successfully, an individual must be able to perform each essential duty satisfactory. Items listed throughout this job description are representative of the knowledge, skills, and abilities required
    • High School graduate or GED
    • Bachelor's degree preferred
    • One (1) year job related experience preferred in senior living communities          
    • Ability to handle multiple priorities
    • Possess written and verbal skills for effective communication and the ability to facilitate small group presentations
    • Competent in organizational, time management skills
    • Demonstrates good judgment, problem solving and decision making skills
    • Ability to make responsible choices and decisions and act in a resident’s best interest
    • Ability to work semi-independently without direct supervision by following community procedures and guidelines and have the ability to follow through on assigned tasks
    • Demonstration of proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel),with the ability to learn new applications
