    DayBridge Adult Day Center, on the beautiful senior campus of The Glenn Hopkins is seeking a highly motivated individual to serve as the Program Coordinator.  Coordinator is responsible for development and implementation of the adult day activities and strategies for participation engagement in the adult day program.  The Program Coordinator works with other Adult Day Service staff to enrich the lives of our clients while they spend their day with us.  This position will work Monday - Friday, and specific hours may vary from week to week.  Great Lakes Management Co. AA/EOE

    Responsibilities

    • Ongoing assessment of current clients, assuring that each client continues to meet Day Service criteria.
    • Maintain the daily attendance roster. Submit to supervisor on a weekly basis.
    • Complete monthly billing and submit to Campus Accountant.
    • Communicate with RN consultant regarding health concerns, care planning and monthly review of health services program in coordination with the Adult Day Director.
    • Understand the clients’ abilities and recreational needs through assessment and plan events and activities based upon that knowledge. Communicate assessed needs to day center staff.
    • Plan, prepare, direct, and participate in a wide variety of recreational activities and events. Assist with daily events.
    • Encourage client attendance and participation in activities.
    • Coordinate transportation services regarding schedule changes, providing transportation resources, communicating with families/clients.
    • Communicate with other staff regarding program issues. (attendance, activities, special events)
    • Maintain record of all meals served for the program for at least six months.
    • Maintain record of all monthly Activity Calendars for the program for at least six months.
    • Attend all required meetings.
    • Participate in professional organizations (Leading Age MN).
    • Assist clients with daily personal care needs (toileting, transferring, meal time assistance, grooming, dressing, etc.) when needed.
    • Assist or perform monthly client vitals check and document appropriately in chart.
    • Other duties as assigned.

    Qualifications

    • Four-year degree in Therapeutic Recreation or related field and/or minimum 2 years experience working in activities in senior services.
    • Effective organization, planning and record keeping skills.
    • Experience with client assessments, initial and on-going, and medical record keeping and narrative charting.
    • Ability to create stimulating programs for senior clients, including those with memory loss.
    • Excellent communication skills, both oral and written.
    • Class B Driver’s license a plus.
    • First Aid and CPR certified.
    • Good work ethic.
    • Maintains confidentiality.
    • Able to read, write, spell, do basic math, speak and understand English.
