    DayBridge, the adult day center on the beautiful campus of The Glenn Hopkins, is looking for a Part-Time Assistant.  The Assistant is responsible under the supervision of the Director of Adult Day Services, and in partnership with other Day Service staff, for personal care needs and activities programming for the clients of the Day Bridge Adult Day Services.  Great Lakes Management Co.     Job Responsibilities and Duties


    • Understand the clients’ abilities and recreational needs and plan events and activities based upon that knowledge to meet assessed interests and needs.
    • Plan, prepare, direct, and lead a wide variety of recreational activities and events. These includes set up and clean up, providing refreshments & snacks, the advertising of events, and the ongoing evaluation of the activity program in coordination with resident abilities and needs.
    • Create a monthly calendar of diversified daily program activities. Including:  socialization activities (group projects, recreational activities), cultivation of personal interests (arts, music, crafts) and activities designed to increase participant’s knowledge & awareness of environment & to enhance language and conceptual skills. 
    • Encourage client participation in program activities.
    • Assist clients with daily personal care needs (toileting, transferring, meal time assistance, grooming, dressing, etc.) when needed.
    • Follow plan of care for each client.
    • Ensure that activity supply expenses remain within the monthly budget.
    • Maintain inventory of supplies and resources for successful programming.
    • Communicate personal care supply needs to Coordinator for reorder/restock.
    • Attend all required meetings and in-services. Complete 8 Hour of education yearly pertaining to adult day program rules and regulations.
    • Assumes leadership role in supervising and coordinating daily program in the absence of the Director.
    • Assist or perform monthly client vitals check and document appropriately in chart.
    • Participate in client care planning process as directed by Program Coordinator.
    • Complete monthly client participation progress note in chart as directed by Day Program Coordinator.
    • Other duties as assigned.


    Physical Requirements (based on essential responsibilities)

    Must be able to frequently perform tasks such as, but not limited to, those listed below:

    • Bend and squat to the floor in using activity program equipment, pick up debris, move small furniture pieces, etc.
    • See objects within 35 feet to monitor work areas and participants attending program.



    Must be able to continuously perform tasks such as, but not limited to, those listed below:

    •  Move, push, pull and/or lift objects weighing 10 – 25 pounds.
    • Use hands, arms and legs for pushing wheelchairs, safe assisted transfers of participants and simple grasping.
    • Perform tasks requiring actions of muscles or groups of muscles to walk, stoop, bend, lift and to carry objects.  
    • Walk and/or stand for prolonged periods of time
    • Speak clearly and make self understood in face-to-face interactions
    • See objects closely to read directions, instructions, perform documentation, etc.



    • Degree in recreational therapy or Nursing Assistant Registered with minimum 2 years experience working in activities in senior housing or related senior area.
    • CPR and First aid certified.
    • Effective organization and planning
    • Ability to create stimulating programs for senior population, including those with memory loss
    • Good work ethic
    • Maintains confidentiality
    • Able to read, write, spell, do basic math, speak and understand English.
